If you wish to pick up at the office during regular office hours, please indicate so in the comment section.
The truancy form expires 30 days after issue.
If this is not the first time you have requested this form, there is a $5 duplicate fee.
A RUSH fee of $15 can be paid to expedite this process. All rushes are processed within 3 business days and mailed Priority.
